چکیده
Persistent lithium-induced neurotoxicity: direct effect of lithium and/or hypernatremia? 7.3.2008 Case report A 54-year-old African-American woman was brought to the emergency department of a university medical center with increased confusion. Over several days prior to presentation, she had become drowsy and disoriented. On the day of her admission she had her clothes on inside out. She did not have any nausea, vomiting or diarrhea, and there were no recent changes in her medications. She was previously diagnosed with bipolar disorder type I, and her medications were lithium carbonate extended-release 450 mg at nighttime, valproic acid 500 mg bid, fluphenazine 30 mg bid, and benztropine 2 mg bid. She had had prior episodes of lithium toxicity. The patient was followed in a community mental health center and, unfortunately, no information was available about how her previous episodes of lithium toxicity were characterized, and more importantly why the lithium was reinstated by her outpatient psychiatrist following these episodes. Physical examination showed a fine resting tremor. The patient was afebrile, with blood pressure of 124/73 mm Hg and pulse of 104 BPM. Initial serum electrolytes were unremarkable (including Na+ of 143 mmol/L), but the creatinine was 2.3 mg/dL. Creatine kinase, ammonia, TSH, folate, vitamin B12 were unremarkable, and WBC count was 12.7 (cells x 1000/microliter). Serum lithium level was 2.4 mEq/L, and valproic acid level was 48 mEq/L.

Persistent lithium-induced neurotoxicity: direct effect of lithium and/or hypernatremia?
Abstract Lithium has been used in the treatment of bipolar affective disorder (BAD) for more than 50 years. Features of lithium toxicity include drowsiness, slurred speech, ataxia, psychomotor slowing, polyneuropathy, impaired memory, seizures, coma and death. Lithium neurotoxicity is usually reversible on cessation of its administration, and irreversible toxicity is uncommon. However, persiste...
متن کاملThe protective effect of curcumin against lithium-induced nephrotoxicity in rats
Lithium is an element which has been used as salts of chloride or carbonate for many years in the treatment of some psychological disorders such as mania, bipolar or schizophrenic diseases. Chronic application of lithium may induce some serious nephropathies such as natriuresis, renal tubular acidosis, tubulointerstitial nephritis progression to progressive chronic kidney disease and hypercalce...
متن کاملLithium-induced nephrogenic diabetes insipidus.
BACKGROUND Lithium can cause nephrogenic diabetes insipidus in up to 20 to 40 percent of patients currently taking the medication, and a subset of these patients will have a persistent concentrating defect long after lithium is discontinued. They are at risk for serious hypernatremia when fluid intake is restricted for any reason. METHODS MEDLINE as used to search the key words "nephrogenic d...
متن کاملGABAA receptors in the dorsal hippocampus are involved in sate-dependent learning induced by lithium in mice
These experiments examined the effects of pre-test intra dorsal hippocampus (intra-CA1) administration of GABAA receptor agonist and antagonist, muscimol and bicuculline respectively, on state-dependent learning induced by lithium. Male NMRI mice were trained in a one-trial step-down inhibitory avoidance task, and immediately after training they received IP injections of either saline (10 mL/kg...
متن کاملDo Antidepressants Lower the Prevalence of Lithium-associated Hypernatremia in the Elderly? A Retrospective Study
BACKGROUND Clinically important measures of lithium-induced nephrogenic diabetes insipidus (NDI) such as hypernatremia have not been well-studied. This is especially relevant for the elderly who, in comparison to younger adults, may become symptomatic and require hospitalization with relatively small elevations in sodium levels. We hypothesized that antidepressant use, which has been associated...
